The Hanson Family Dental Comprehensive Care Plan for Periodontal Maintenance is designed to make high-quality care more accessible and cost-effective to those patients that need essential Periodontal Maintenance treatments.

This is not dental insurance, but a savings plan. There are no claim forms, no waiting periods, no “missing tooth”clause and no need for pre-treatment estimates.

• 4 Perio Maintenance Cleanings (D4910) • 2 Routine Exams (D0120) • 1 Urgent Care Exam (D0140) • 4 Fluoride Treatments (D1206)  • All Necessary Diagnostic X-Rays (D0210,D0272, D0274, D0330, excluding D0367) • 15% OFF additional services, including in-office specialty treatments

Comprehensive Care Plan for Periodontal Maintenance Annual Membership Cost*:
Individual/First Member- $549
Additional Family Member – $499

Comprehensive Care Plan for Periodontal Maintenance begins immediately at time of enrollment and is effective for one calendar year.

This is not dental insurance, but a savings plan only valid at Hanson Family Dental located at 604 County Rd, Hanson, MA. Cannot be combined with any dental insurance plan. Membership begins at time of enrollment and continues for one calendar year. Either the member or Hanson Family Dental reserves the right to terminate the plan. This membership is personal to the individual and cannot be transferred or refunded. All additional services must be paid for at time of service. Excludes In-Office Merchandise (Electric Toothbrushes and Toothpaste) and Whitening products.
